Primary Care Fellowship for Advanced Pharmacist Practitioner (APP)

Wynn Medical Center is seeking a dedicated and highly skilled Advanced Pharmacist Practitioner (APP) Fellow to join our dynamic team in 2025-2026 Academic Year.

This fellowship offers a unique opportunity to integrate clinical pharmacy practice with patient education, medication management, clinical research, and inter-professional training.

Fellow's Responsibilities:

  1. Clinical Pharmacy Practice:

    • Educate patients about their chronic medical conditions medications, including proper usage, potential side effects, and expected outcomes.

    • Conduct medication reconciliation to ensure accuracy and safety in medication management.

    • Perform follow-up calls to monitor high risk patient medication adherence and provide necessary interventions.

    • Oversee and manage the i-Pledge Program for Isotretinoin acne treatment, ensuring compliance with all regulatory requirements.

  2. Patient Care:

    • Assist attending physicians in the management of patients with chronic conditions by providing pharmaceutical expertise and patient counseling.

    • Develop and implement individualized medication plans to optimize therapeutic outcomes.

    • Follow-up and monitor acne patients for iPledge program 

    • Follow-up Rheumatology/Dermatology biologic patients for counseling/injection

    • Monitor lab and imaging results, discuss w/ attending physicians promptly for abnormal values 

  3. Education and Training other Trainees:

    • Teaching PharmD/MD students. The fellow will coordinate with pharmacy and medical school to design and deliver training programs for PharmD and MD students.

    • Mentor and precept students during their clinical rotations, fostering a collaborative learning environment.

    • Coordinate with accredited agencies to initiate the accreditation process for WMC fellowship and residency programs. 

  4. Clinical Research:

    • Participate In clinical research projects aimed at evaluating the efficacy and safety of medications at WMC clinics. 

    • Collaborate with interdisciplinary teams to design, conduct, and publish research studies.

    • Conduct monthly research meetings with general manager, clinicians, and WMC research staff assistants (Kloe Truong, Nhi Truong)

  5. Interprofessional Collaboration:

    • Work closely with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care.

    • Contribute to the development of clinical protocols and guidelines to improve patient outcomes.
       

Qualifications:

  • Education:

    • Doctor of Pharmacy (PharmD) degree from an accredited institution.

    • Completion of a PGY-1 Pharmacy Practice Residency and PGY-2 Specialty Residency preferred.

  • Licensure/Certification:

    • Active pharmacist license in the state of practice.

    • Certification in relevant clinical specialties (e.g., BCPS, BCACP) is desirable.

    • APP licensure eligibility

  • Experience:

    • Minimum of 1 years of clinical pharmacy experience in a hospital or outpatient setting.

    • Experience with patient education, medication reconciliation, and adherence monitoring.

    • Direct patient contact

  • Skills:

    • Strong clinical knowledge and ability to apply evidence-based practices.

    • Excellent communication and interpersonal skills to effectively educate and collaborate with patients and healthcare professionals.

    • Proficiency in conducting clinical research and data analysis.

    • Ability to mentor and train students and residents.

Fellowship Curriculum
 

Month 1-4: Internal Medicine/Family Medicine Rotation (Rosemead Office)
Objective:
To develop comprehensive skills in managing common acute and chronic conditions in adults, including hypertension, diabetes, cardiovascular disease, and respiratory conditions.

Core Components:

  • Clinical Management: Focus on diagnosis, management, and treatment of chronic diseases (e.g., hypertension, diabetes, hyperlipidemia).

  • Preventive Care: Emphasis on preventive health measures, including vaccinations, screening tests, and lifestyle counseling.

  • Pharmacotherapy Optimization: Review and manage polypharmacy, drug interactions, and medication adherence.

  • Patient Education: Provide patient education on disease management, medication use, and lifestyle modifications.

Learning Activities:

  • Weekly Case Conferences: Discuss complex cases with a multidisciplinary team.

  • Patient Rounds: Participate in daily rounds, contributing to treatment planning and follow-up.

  • Clinical Simulations: Engage in simulated scenarios for acute care situations.

Evaluation:

  • Clinical Performance: Assessed by preceptors through direct observation and feedback.

  • Case Presentations: Present a comprehensive case at the end of the rotation.

  • Reflection Paper: Submit a reflection on the integration of pharmacotherapy in primary care.

Month 5-6: Nephrology Rotation (Rosemead Office)

Objective:
To gain expertise in the management of kidney-related disorders and their pharmacological treatments, including chronic kidney disease (CKD) and electrolyte imbalances.

Core Components:

  • CKD Management: Understand the stages of CKD, related complications, and their management.

  • Dialysis Pharmacotherapy: Learn about medication management in dialysis patients, including dose adjustments.

  • Electrolyte Management: Focus on the management of electrolyte disturbances (e.g., hyperkalemia, hyponatremia).

  • Hypertension in Nephrology: Specialized focus on managing hypertension in patients with kidney disease.

Learning Activities:

  • Interdisciplinary Rounds: Participate in nephrology rounds, focusing on pharmacotherapy in renal patients.

  • Journal Club: Review and present recent nephrology-related pharmacotherapy research.

  • Patient Consults: Conduct medication reviews and provide recommendations for nephrology patients.

Evaluation:

  • Clinical Performance: Continuous assessment by nephrology preceptors.

  • Research Presentation: Present findings from a nephrology-related research topic.

  • Objective Structured Clinical Examination (OSCE): Demonstrate competence in managing renal pharmacotherapy scenarios.

Month 6-7: Pediatrics Rotation (Monterey Park Office)

Objective:
To develop skills in pediatric pharmacotherapy, focusing on age-appropriate medication management, dosing, and patient education.

Core Components:

  • Pediatric Pharmacotherapy: Learn about dosing calculations, formulations, and safety considerations in pediatrics.

  • Acute Pediatric Conditions: Manage common acute pediatric conditions (e.g., respiratory infections, gastroenteritis).

  • Chronic Pediatric Conditions: Focus on managing chronic pediatric conditions such as asthma, ADHD, and diabetes.

  • Vaccination: Gain proficiency in pediatric immunization schedules and counseling.

Learning Activities:

  • Pediatric Rounds: Participate in daily pediatric rounds with a focus on pharmacotherapy.

  • Patient Education Workshops: Develop and lead workshops for parents on medication safety and adherence.

  • Simulation Labs: Practice emergency pediatric scenarios, including anaphylaxis and seizure management.

Evaluation:

  • Case Log: Maintain a log of pediatric cases with detailed pharmacotherapy plans.

  • Peer Teaching: Present pediatric pharmacotherapy topics to peers.

  • End-of-Rotation Exam: Comprehensive written and practical exam covering pediatric pharmacotherapy.

Month 8-9: Rheumatology/Dermatology Rotation (Rosemead Office)

Objective:
To specialize in the pharmacological management of rheumatologic and dermatologic conditions, with a focus on biologics, immunosuppressants, and patient monitoring.

Core Components:

  • Rheumatology Pharmacotherapy: Learn about the management of rheumatoid arthritis, lupus, and gout.

  • Dermatology Pharmacotherapy: Focus on the treatment of chronic skin conditions such as psoriasis, eczema, and acne.

  • Biologics and Immunosuppressants: Understand the use, monitoring, and adverse effects of biologics and immunosuppressants.

  • Patient Monitoring: Develop skills in monitoring for side effects and managing therapy adjustments.

Learning Activities:

  • Case Discussions: Participate in interdisciplinary case discussions focusing on complex rheumatology and dermatology cases.

  • Clinical Consultations: Provide medication recommendations and monitoring plans for rheumatology/dermatology patients.

  • Literature Review: Review current guidelines and evidence-based practices in rheumatology and dermatology.

Evaluation:

  • Case Report: Submit a detailed case report on a patient managed during the rotation.

  • Clinical Performance: Assessed through direct patient care activities and feedback from preceptors.

  • OSCE: Demonstrate proficiency in managing rheumatologic and dermatologic pharmacotherapy scenarios.

Month 10-11: Psychiatry Rotation (Rosemead Office with Distance Learning from UCI Psychiatry Fellowship Program)

Objective:
To build competence in the pharmacological treatment of psychiatric disorders, including mood disorders, anxiety, psychosis, and substance use disorders.

Core Components:

  • Psychopharmacology: Focus on the pharmacotherapy of depression, anxiety, bipolar disorder, schizophrenia, and substance use disorders.

  • Medication Management: Learn about the initiation, titration, and monitoring of psychiatric medications.

  • Patient Counseling: Develop skills in counseling patients and families about psychiatric medications and potential side effects.

  • Interprofessional Collaboration: Work closely with psychiatrists, psychologists, and social workers in a collaborative care model.

Learning Activities:

  • Patient Consults: Conduct and present medication reviews for psychiatric patients.

  • Behavioral Health Rounds: Participate in behavioral health team meetings, contributing pharmacotherapy insights.

  • Patient Education Sessions: Lead sessions on medication adherence and managing side effects.

Evaluation:

  • Reflection Paper: Reflect on the challenges and successes in managing psychiatric patients.

  • Peer Review: Participate in peer reviews of case management strategies.

  • Final Presentation: Present a comprehensive case involving complex psychopharmacology to the fellowship cohort.

Month 12: Capstone and Elective Experience

Objective:
To synthesize learning from the entire fellowship, focusing on a specific area of interest or further developing expertise in a chosen specialty.

Core Components:

  • Elective Focus: Choose an elective in any of the prior rotations or a related area of interest, such as oncology, infectious disease, or geriatrics.

  • Capstone Project: Develop and present a capstone project that addresses a clinical challenge or advances practice within the chosen elective area.

  • Advanced Clinical Practice: Engage in more autonomous clinical practice under the guidance of a preceptor, focusing on leadership and decision-making skills.

Learning Activities:

  • Project Development: Work on a capstone project that could involve research, quality improvement, or educational initiatives.

  • Advanced Patient Care: Provide comprehensive care to a panel of patients, integrating pharmacotherapy, patient education, and interprofessional collaboration.

  • Mentorship: Receive mentorship from a senior clinician in the chosen area of focus.

Evaluation:

  • Capstone Presentation: Present the capstone project to a panel of faculty and peers.

  • Clinical Competency Assessment: Final assessment of clinical skills across all rotations, focusing on areas of strength and improvement.

  • Program Reflection: Submit a reflection on the fellowship experience, highlighting key learning points and areas for future growth.
