Primary Care Fellowship for Advanced Pharmacist Practitioner (APP)
Wynn Medical Center is seeking a dedicated and highly skilled Advanced Pharmacist Practitioner (APP) Fellow to join our dynamic team in 2025-2026 Academic Year.
This fellowship offers a unique opportunity to integrate clinical pharmacy practice with patient education, medication management, clinical research, and inter-professional training.

Fellow's Responsibilities:

Clinical Pharmacy Practice:
-
Educate patients about their chronic medical conditions medications, including proper usage, potential side effects, and expected outcomes.
-
Conduct medication reconciliation to ensure accuracy and safety in medication management.
-
Perform follow-up calls to monitor high risk patient medication adherence and provide necessary interventions.
-
Oversee and manage the i-Pledge Program for Isotretinoin acne treatment, ensuring compliance with all regulatory requirements.
-

Patient Care:
-
Assist attending physicians in the management of patients with chronic conditions by providing pharmaceutical expertise and patient counseling.
-
Develop and implement individualized medication plans to optimize therapeutic outcomes.
-
Follow-up and monitor acne patients for iPledge program
-
Follow-up Rheumatology/Dermatology biologic patients for counseling/injection
-
Monitor lab and imaging results, discuss w/ attending physicians promptly for abnormal values
-

Education and Training other Trainees:
-
Teaching PharmD/MD students. The fellow will coordinate with pharmacy and medical school to design and deliver training programs for PharmD and MD students.

Mentor and precept students during their clinical rotations, fostering a collaborative learning environment.

Coordinate with accredited agencies to initiate the accreditation process for WMC fellowship and residency programs.

Clinical Research:
-
Participate In clinical research projects aimed at evaluating the efficacy and safety of medications at WMC clinics.
-
Collaborate with interdisciplinary teams to design, conduct, and publish research studies.
-
Conduct monthly research meetings with general manager, clinicians, and WMC research staff assistants (Kloe Truong, Nhi Truong)

Interprofessional Collaboration:

Work closely with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care.

Contribute to the development of clinical protocols and guidelines to improve patient outcomes.

Qualifications:
-
Education:
-
Doctor of Pharmacy (PharmD) degree from an accredited institution.
-
Completion of a PGY-1 Pharmacy Practice Residency and PGY-2 Specialty Residency preferred.

Licensure/Certification:
-
Active pharmacist license in the state of practice.
-
Certification in relevant clinical specialties (e.g., BCPS, BCACP) is desirable.

Experience:

Minimum of 1 years of clinical pharmacy experience in a hospital or outpatient setting.

Experience with patient education, medication reconciliation, and adherence monitoring.

Fellowship Curriculum
Month 1-4: Internal Medicine/Family Medicine Rotation (Rosemead Office)
Objective:
To develop comprehensive skills in managing common acute and chronic conditions in adults, including hypertension, diabetes, cardiovascular disease, and respiratory conditions.
Core Components:

Clinical Management: Focus on diagnosis, management, and treatment of chronic diseases (e.g., hypertension, diabetes, hyperlipidemia).

Preventive Care: Emphasis on preventive health measures, including vaccinations, screening tests, and lifestyle counseling.

Pharmacotherapy Optimization: Review and manage polypharmacy, drug interactions, and medication adherence.

Patient Education: Provide patient education on disease management, medication use, and lifestyle modifications.

Month 5-6: Nephrology Rotation (Rosemead Office)
Objective:
To gain expertise in the management of kidney-related disorders and their pharmacological treatments, including chronic kidney disease (CKD) and electrolyte imbalances.
Core Components:

CKD Management: Understand the stages of CKD, related complications, and their management.

Dialysis Pharmacotherapy: Learn about medication management in dialysis patients, including dose adjustments.

Electrolyte Management: Focus on the management of electrolyte disturbances (e.g., hyperkalemia, hyponatremia).

Hypertension in Nephrology: Specialized focus on managing hypertension in patients with kidney disease.

Acute Pediatric Conditions: Manage common acute pediatric conditions (e.g., respiratory infections, gastroenteritis).

Rheumatology Pharmacotherapy: Learn about the management of rheumatoid arthritis, lupus, and gout.

Dermatology Pharmacotherapy: Focus on the treatment of chronic skin conditions such as psoriasis, eczema, and acne.

Biologics and Immunosuppressants: Understand the use, monitoring, and adverse effects of biologics and immunosuppressants.

Objective:
To build competence in the pharmacological treatment of psychiatric disorders, including mood disorders, anxiety, psychosis, and substance use disorders.
Core Components:

Psychopharmacology: Focus on the pharmacotherapy of depression, anxiety, bipolar disorder, schizophrenia, and substance use disorders.

Medication Management: Learn about the initiation, titration, and monitoring of psychiatric medications.

Patient Counseling: Develop skills in counseling patients and families about psychiatric medications and potential side effects.

Elective Focus: Choose an elective in any of the prior rotations or a related area of interest, such as oncology, infectious disease, or geriatrics.

Capstone Project: Develop and present a capstone project that addresses a clinical challenge or advances practice within the chosen elective area.

Advanced Clinical Practice: Engage in more autonomous clinical practice under the guidance of a preceptor, focusing on leadership and decision-making skills.

Project Development: Work on a capstone project that could involve research, quality improvement, or educational initiatives.

Advanced Patient Care: Provide comprehensive care to a panel of patients, integrating pharmacotherapy, patient education, and interprofessional collaboration.
